Use this cat medical tracker to keep track of your canine’s vaccinations, weight, flea/tick treatments, heartworm treatments, and more! Great for emergencies or even just to remember when your pet’s last vet visit was.
I decided to make this after a recent visit to the vet. I got my cats at two different times and I can never remember which one has which immunization and such. So I took my oldest to the vet to get her rabies renewed.
Instead, I wound up paying about $120 for my vet to tell me, “Oh, your cat is already up-to-date on her immunizations.”
Well, why didn’t you tell me that on the phone when I told you I wanted an appointment explicitly for this purpose? Ugh. (They used to call me when the cats needed to be updated, but I guess they decided they could just trick us into paying for nothing by simply not doing that anymore. My younger cat did need his vaccinations renewed and I never received a call.)
No more of that! Now I know when and what my cat received, as well as any at-home treatments I’ve administered (such as flea medications).
Free Cat Medical Tracker Printable
It folds up into a nice brochure that fits easily into a purse or into your kitty carrier. In fact, I recommend just keeping it in the carrier. That way you don’t have to hunt for it if you have an emergency and need to rush to the vet.
On the front, you can keep track of general information, like your cat’s age and microchip number. (This is also handy just in case your cat gets lost.)
On the inside, you can mark the date and which immunizations were received, such as rabies and distemper. There are also spaces to keep track of heart-worm, flea, and other parasitic medications.
On the back are place to track age and weight. There is also a handy informative section about the common diseases your cat should be protected against. It also lists the symptoms.
This is a great pamphlet to have on hand in case of a kitty emergency. Especially if you travel with your pet and you need to see a vet that doesn’t have access to your pet’s medical records. Fill it out and keep it inside your pet carrier so you’ll always have it while traveling. That’s one less thing to have to worry about if you’re in a hurry.

Have a vet look over any pet you wish to adopt. Veterinarians will stamp their medical records with their information (name, address, and phone number) to advertise their business and to make sure you have it in the case of emergency. If you received an unstamped version of this from someone else, it did not come from a vet.
Always ask for the information of the vet that any private seller claims they used. If they can’t give you the information, walk away. If they can, call that vet and ask that they corroborate the seller’s story.
Please use common sense when adopting a pet.

We received this kitten version of your medical records when we bought a kitten from someone claiming that she just bought a kitten but her son was very allergic and needed to find a new home for her. The pamphlet showed that the kitten had been dewormed and had its 1st vaccination shot just 2 days earlier. The kitten was tiny and didn’t seem well but the seller told us that she just had her shots and the vet said she may act like that for a day or two and showed us this records dated and checked off for the vaccine. Well the kitten died within 24 hrs. I doubt the kitten even saw a vet now. We thought the records were from a vet but looking into it i see the seller just downloaded this and marked it off herself. I think your vaccination records is a great idea but I think you need to add something to it so that people know it’s not an official record from a vet.
That’s very sad about your kitten, Andrew. I’ll go through and add a watermark when I get the chance. Unfortunately, I can’t put something like, “This is not from a vet” on it because I know many vet offices download and use my tracker. It’s unfortunate that someone used it improperly like that. But she would have been able to do that with one from a vet, as well. (It’s very easy to get a medical record from them and then mark things on your own.)
Just so you know, vets put their own information on the medical records that they give out. If you’re given a record without a stamp with the vet’s name, number, and address, it is not from a vet.
